Patient Care

Providing comprehensive nursing care together with support for carers and families for people with a life-limiting illness. Our focus is to help maintain independence, dignity and maximize quality of life.

This includes:

  • Pain and symptom management

  • General and specialist nursing care

  • Advice and emotional support

  • Loan of specialised equipment in the home

The Nurses assess each individual’s needs, working closely with the doctor and family to provide a coordinated, and personalised approach to management of care.

How can I be referred to the Palliative Care Service?

Referral to the Central Grampians Palliative Care Service can be made by anyone involved with the patient’s care. The doctor or nursing staff can refer, as well a family member, or friend, as long as they have the patient’s consent. The Palliative Care Nurse will come to the referee’s home and complete the referral or will visit the patient whilst in hospital.

Patient Care

Central Grampians Health Palliative Care Service looks after patients either as outpatients or as inpatients within the Rural City of Ararat, Northern Grampians Shire and Pyrenees Shire. Nursing care is managed in conjunction with your existing medical team including GP’s, Medical and Radiation Oncologists, Surgeons and Physicians.

Our Home Based Nursing Service operates 5 days a week from 8.30 – 4.30

After hours support is available if required via District Nursing.

Referral and Liaison with other services

At no charge to the client or family services can include:

  • Carer respite

  • Social workers

  • Counselling and bereavement support

  • Allied Health support

  • Spiritual and pastoral care

  • Palliative Care Physicians

For more complex symptom management and assessment, both the Grampians Regional Palliative Care Team and specialist in-patient care can also be accessed through Central Grampians Palliative Care

Referrals can be made to District Nursing, Home Care services, Meals on Wheels and other HACC Community Support Services as required. These services may charge a small fee at the discretion of each service.
