• Ballarat Hospice Care Inc.
• Ballarat Health Service - Regional Palliative Care Team - Gandarra Inpatient Unit
• St John of God
• Central Grampians Palliative Care
• Wimmera Hospice Care

The Department of Health funds a Consortium Manager whose role it is to assist the services provide a best practice service.
